Common concerns

Depression and anxiety may appear as sadness, fear, irritability, low energy, sleep disturbance, physical complaints or difficulty functioning. Some women experience severe premenstrual mood symptoms, depression or anxiety during pregnancy or after childbirth, or mood changes around menopause. Domestic violence, infertility, pregnancy loss and unequal caregiving burdens can also affect mental health.

After childbirth: baby blues or something more?

Mild tearfulness and emotional change can occur soon after delivery. Symptoms that are intense, last, affect sleep even when the baby sleeps, create hopelessness, interfere with bonding, or include thoughts of harm need prompt assessment. Confusion, extreme agitation, little need for sleep, hallucinations or unusual beliefs after childbirth are an emergency.

Treatment and reproductive safety

Care may include psychological therapy, social support, treatment of sleep problems and medication. Pregnancy, breastfeeding, menstrual patterns and contraception can affect decisions. Do not stop psychiatric medicine suddenly because of pregnancy without speaking to the prescriber; both untreated illness and medicines need an individual risk-benefit discussion.

A respectful assessment

A clinician may ask about mood, anxiety, trauma, relationships, safety at home, physical health, thyroid symptoms, medicines and reproductive stage. A woman can ask to speak privately. Abuse is never the victim’s fault.

Preparing for a consultation

A clear history helps the doctor understand what has changed and what support may be appropriate. If possible, bring a trusted family member and:

  • how symptoms relate to periods, pregnancy, birth or menopause
  • sleep, appetite, functioning and safety changes
  • all medicines and supplements, including pregnancy or breastfeeding status
  • support available at home and any relationship-safety concerns
