Changes that deserve assessment
Warning signs include repeating questions, getting lost in familiar places, difficulty managing money or medicines, word-finding problems, poor judgement, personality change or struggling with familiar tasks. Depression, sleep problems, medication effects, vitamin or thyroid problems, infection, seizures and stroke can also affect thinking, so guessing the cause is unsafe.
What evaluation may involve
The clinician asks the person and someone who knows them well about the timeline, daily abilities, behaviour, medicines and health. Memory and neurological testing, physical examination, blood tests or brain imaging may be recommended. Sudden confusion developing over hours or days is not typical ageing and needs urgent medical assessment.
Treatment and ongoing care
Treatment depends on the cause and type. Some contributors can be treated. For dementia, care may include medicines for selected patients, management of mood or behaviour, physical-health care, routines, activity, hearing or vision support and caregiver education. No single approach cures every dementia.
Practical family planning
Support independence where safe while simplifying medicines, finances, cooking and travel. Discuss driving, falls, wandering and emergency contacts. Early conversations about legal, financial and future-care preferences allow the person to participate. Caregivers also need rest and support.
Preparing for a consultation
A clear history helps the doctor understand what has changed and what support may be appropriate. If possible, bring a trusted family member and:
- a family member who knows the person’s usual functioning
- a timeline and examples of daily tasks becoming difficult
- all medicines, hearing aids, glasses and previous reports
- information about falls, wandering, driving and home safety
