Patient & family guide

Psychosis affects how a person understands reality. Early, respectful assessment can improve safety and help families begin appropriate care.

Please note: This guide explains common patterns. It cannot determine what is happening to a particular person, and it does not replace medical assessment.

At a glanceFour areas clinicians assessSymptoms vary; a person does not need every feature.

- Perception Hearing or seeing things others do not
- Beliefs Strong fixed beliefs despite contrary evidence
- Thinking Confused speech or difficulty organising thoughts
- Function Withdrawal, low motivation or reduced self-care

## What psychosis can look like

A person may hear voices, feel watched or controlled, hold strong beliefs others do not share, speak in a difficult-to-follow way, or behave unusually. Other symptoms are quieter: reduced expression, withdrawal, low motivation and difficulty planning. These experiences are real and frightening to the person.

Psychosis is a symptom, not automatically schizophrenia. It can occur with bipolar disorder, severe depression, substance use, some medicines, seizures, infections and other medical conditions.

## Assessment and treatment

Assessment usually covers the onset and course of symptoms, safety, sleep, mood, substance use, physical and neurological symptoms, medicines and family observations. Tests may be needed to look for medical causes.

Treatment may include antipsychotic medication, education, family work, rehabilitation and support with sleep, daily routine and social functioning. Monitoring side effects and physical health is important. Recovery goals should reflect the person’s priorities and dignity.

## Talking with someone who has unusual beliefs

Stay calm, use short sentences and acknowledge the fear without confirming the belief—for example, “I can see this feels frightening.” Avoid mocking, crowding or trying to win an argument. Offer help contacting the clinician. Reduce access to weapons or dangerous items if risk is rising.

## When care becomes urgent

Urgent evaluation is needed for threats, command hallucinations telling someone to harm, severe agitation, inability to eat or drink, wandering into danger, catatonia, sudden confusion, or symptoms beginning after substance use or a medical illness.

Get urgent help now: Call India’s emergency number 112 or go to the nearest hospital emergency department for immediate danger, a suicide attempt, serious self-harm risk, violence, unconsciousness, seizure lasting five minutes or repeated seizures, overdose, poisoning, severe withdrawal, sudden weakness or speech difficulty, or severe confusion. Tele-MANAS offers 24×7 mental-health support at 14416.

## Preparing for a consultation

A clear history helps the doctor understand what has changed and what support may be appropriate. If possible, bring a trusted family member and:

- when unusual experiences began and how they changed
- videos or notes of concerning behaviour when safely available
- all medicines, substances and recent illnesses
- information about sleep, eating, self-care and safety
