Patient & family guide

OCD is not simply liking cleanliness. It involves unwanted thoughts, images or urges and repeated actions or mental rituals that become difficult to control.

Please note: This guide explains common patterns. It cannot determine what is happening to a particular person, and it does not replace medical assessment.

At a glanceHow the OCD loop continuesThe ritual reduces distress briefly, which teaches the brain to repeat it.

- Obsession An unwanted thought, image, doubt or urge
- Distress Anxiety, guilt, disgust or uncertainty
- Compulsion Checking, washing, repeating or a mental ritual
- Brief relief Fear returns and the loop starts again

## What OCD may look like

Common themes include contamination, accidental harm, religion, morality, sexuality, symmetry or doubt. Compulsions may be visible—washing, arranging, checking—or hidden, such as counting, repeating words, reviewing memories or seeking reassurance. Having an intrusive thought does not mean a person wants to act on it.

Many people know their fears are excessive yet still feel driven to perform rituals. Shame can delay help, especially when the thoughts involve taboo subjects.

## When to seek assessment

Assessment is useful when obsessions or rituals take significant time, cause distress, damage skin, delay leaving home, interrupt prayer or study, affect relationships, or make family members participate. The clinician also checks for depression, tics, anxiety, psychosis and medical or substance-related causes.

## Treatment and family support

A well-studied psychological treatment is cognitive behavioural therapy using exposure and response prevention: the person gradually faces a trigger while learning not to perform the ritual. Medication may also be considered. The pace and plan should be set with a trained professional; forcing sudden exposure at home can be overwhelming.

Families can respond kindly without joining every ritual. Agreeing on small limits and praising effort is usually more helpful than arguing about whether a fear is logical.

## What OCD is not

OCD is not a character flaw, lack of faith or proof that someone is dangerous. It also differs from ordinary preferences and from delusions. Only a proper assessment can clarify the diagnosis.

Get urgent help now: Call India’s emergency number 112 or go to the nearest hospital emergency department for immediate danger, a suicide attempt, serious self-harm risk, violence, unconsciousness, seizure lasting five minutes or repeated seizures, overdose, poisoning, severe withdrawal, sudden weakness or speech difficulty, or severe confusion. Tele-MANAS offers 24×7 mental-health support at 14416.

## Preparing for a consultation

A clear history helps the doctor understand what has changed and what support may be appropriate. If possible, bring a trusted family member and:

- examples of intrusive thoughts and rituals, including mental rituals
- roughly how much time they take each day
- situations being avoided and ways family members are involved
- previous therapy, medicines and response
